Pre-Transport Support Guidelines
Baseline Pre-Transport Support - within one hour
- Inhalation injury or large % BSA; intubate if requested by receiving hospital; give high flow oxygen to maintain oxygen saturation of greater than 92% with humidification.
- Calculate fluid resuscitation from time of injury as Time 0; give 1/2 calculated volume in first 8 hours; 2nd 1/2 volume within the remaining 16 hours based on the following:
- 2.0 - 4.0 cc Ringers Lactate X % BSA X Lean Weight per 24 hours
- monitor fluid balance (will require a Foley catheter)
- perform baseline investigations
- perform escharotomies on circumferential deep 2nd or 3rd degree burns if nerve or circulatory supply is compromised
- keep the patient comfortable with small repeated boluses of intravenous analgesics given on a PRN basis
- cover the affected area with a sterile sheet; assist to keep the affected body part elevated to reduce edema if not in shock
If transport to a specialized facility is indicated and undertaken, the following support is required:
