Fluid Resuscitation - Hospital Treatment
Undertake IV fluid resuscitation if:
- % BSA is greater than 20% in otherwise healthy individuals
- % BSA is greater than 10% in young, elderly, or those with compromising medical conditions
Modified Parkland Formula
- 2.0 - 4.0 ccs Ringers Lactate X % BSA X Lean Weight per 24 hours
- calculate deficit from the time of the injury and replac eit using this as Time Zero.
- Give 1/2 the calculated volume in the first 8 hours, and the 2nd half over the remaining 16 hours.
- Children require maintenance fluids in addition to therapeutic requirements, based on the following:
- Less than 10 kg - 4.0 cc/kg/hr
- 10 - 20 kg - 2.0 cc/kg/hr
- over 20 kg - 1 cc/kg/hr
- Infants are very prone to hypoglycemia due to limited glycogen stores and therefore should be resuscitated with D5RL as well as having frequent glucometer checks.
Note: All patients requiring resuscitation require Foley Catheter.
