Fluid Resuscitation Pre-Transport - First Four Hours
Start IV fluid resuscitation if:
- %BSA is greater than 20 % in otherwise healthy individuals.
- %BSA is greater than 10 % in young, elderly or those with compromising medical conditions.
Modified Parkland Formula
- Deficit is calculated from the time of injury and is replaced using this as Time 0;
- 1/2 the calculated volume is given in the first 8 hours and the 2nd half is given over the remaining 16 hours;
- 2-4 ccs Ringers Lactate X % BSA X Lean Weight per 24 hours
- in children, maintenance fluids are required in addition to therapeutic requirements, based on the following:
- Less than 10 kg - 4 cc/kg/hr
- 10 - 20 kg - 2 cc/kg/hr
- greater than 20 kg - 1 cc/kg/hr
- Infants are very prone to hypoglycemia due to limited glycogen stores and therefore should be resuscitated with D5RL as well as having frequent glucometer checks.
Note: All patients requiring resuscitation require Foley catheter.
